Chaffee County Public Health is asking community members to participate in the 2026 Community Health Assessment.
Residents are invited to complete a confidential community survey that will help identify the health issues, strengths, and priorities that matter most. The survey is available in both English and Spanish and takes approximately 10–15 minutes to complete.
Individual responses are confidential and will only be reported as part of summarized community data.
Results from the assessment will be shared publicly and used to develop Chaffee County’s next
Community Health Improvement Plan (CHIP).
In Colorado, every five years local public health agencies are required to access the health needs of their communities and use that information to guide planning and decision making.
